***CAMPERS WILL NOT BE ALLOWED TO PARTICIPATE IN CAMP IF THEY DO NOT BRING THIS COMPLETED FORM. THERE WILL BE NO EXCEPTIONS.***
Please take note of the following information that is REQUIRED for the Medical Waiver:
- Camper's parent or legal guardian signature
- A physician's signature releasing the camper to participate in camp activities (in the space provided on the waiver)
A copy camper's physical dated on or after June, 2012. (The physical must be dated within one year prior to start the camp.) A copy of a physical used for the camper's school athletic activities is acceptable. (Please do not bring the original physical, you will not get it back.)
- Athletic Trainers will be available at camp check-in and at all practice sessions to discuss any questions or concerns regarding camper's medication or previous/existing injuries. They will also be available in the residence hall at night. Staff trainers refer medical emergencies and illnesses to Bryn Mawr Hospital in nearby Bryn Mawr. Children under eighteen years of age may only received medical attention if a parent or legal guardian has signed the accompanying release.
